We strive to coordinate care with professional communication and provide ongoing feedback about shared clients. We are very focused on high-quality coordination of care.

PSYCHOLOGICAL EVALUATION REFERRALS
Primary care physicians and therapists who would like to refer their patients to Parker Collins for a psychological evaluation are encouraged to review our External Referral Policy and complete the External Referral Form available below. Please call our office staff at 651-604-7771 and press 2 with questions about the referral process. It is recommended that individuals verify their insurance coverage for assessment prior to scheduling intake. We do have a waiting list for assessment.
